Sudbury's Centre For Transitional Care

The objectives of Sudbury Supportive Housing, formerly Sudbury’s Centre for Transitional Care, are to build capacity within the homeless serving sector. We use service integration methods to make resources provided by independent and government-run organizations readily accessible. The goal of integrating these services is to meet each individual where they are at, complete a risk assessment, and develop a personalized plan to support individuals in supporting themselves and dependents.
